Contract
Each Seminar
costs $_______ plus mileage, transportation, accommodations, and per diem fees if
applicable. Purchase order or contract must be completed and signed 60 days
prior to seminar. Contracting party will make request for purchase order. One
half fee due with signed contract at least 60 days before seminar, fee balance
due before presentation.
Mileage Fee - All automobile
mileage billed at 32 cents per mile, for all mileage to, from, and during
seminars. Fees to be paid within 30 days of receiving receipts.
Transportation - All airfare, and
other transportation fees to, from, and during seminars will be paid by
contracting party. Fees to be paid within 30 days of receiving receipts.
Accommodations - All costs for
accommodations while in transport to, from, and during seminar will be paid by
contracting party. Fees to be paid within 30 days of receiving receipts.
Per diem - A per diem of $50.00
will be paid per day, to cover meals while in transport to, from, and during
seminars. Fees to be paid within 30 days of date of seminar.
Fees must be paid in the form of
company check, Visa or Mastercard.
Seminar instructors (Paul Haider
and other instructors) retain the right to sell books, tapes and other materials
during seminars, and fees collected from such sales are separate from contracted
fees for services for seminar presentations.
All materials, handouts, books,
tapes etc. are under copyright by Paul Haider and may not be used or copied
without prior written permission.
All fees uncollected and past due
(over 30 days) will incur a 1.5 percent surcharge per month, 18% APR.
All seminar cancellations must be
made 30 days in advance of seminar.
Contracting party canceling
seminars with less than 30 days notice will be responsible for payment of half
the seminar fee, plus any other fees incured. Contracting party agrees to all
terms of contract as stated in fee schedule.
